Home Health Advantage recognizes that patients are all people that have individualized needs that must be met in order to be successful in their recovery at home.
The following are signs or symptoms that we try to identify upon admission into our care that are considered Risk Factors that can be predictors of returning to the hospital:
- History of falls
- Unintentional weight loss
- Unexplained exhaustion
- History of multiple hospitalizations
- Decline in mental, emotional or behavioral status
- Currently taking 5 or more medications
- History of multiple visits to the Emergency Department
- Difficulty following medical instructions
- Social and Financial issues
Upon identification of any of these risk factors, Home Health Advantage will work with you to develop an individualized Care Plan to reduce these risks now that you are home.
