Social Worker

Home Health Social Workers assist the clinical team with transitioning patients back into their everyday lives with the support necessary. Our goal is to provide support and decreasing our services over time. The ultimate goal is to allow our patients to live as independently as possible with minimal support.

Social Workers can assist with applications, make phone calls, and provide information and direction by walking patients and families through their needs. Social Workers suggest community resources and how to obtain them. Home health is short term so the social workers will be visiting in person or over the phone coordinating care 1-2 times. Social Workers provide emotional support and community resources as long as there is another skill such as a nurse or therapist coming to visit; which makes it that much more important for patients and/or families to do their part in obtaining longer-term resources.
