We can take an unlimited amount of traditional Medicare. We are in network with some substitute Medicare plans (please fax over information, and we will verify in network coverage).
To qualify for Medicare’s home health care benefit, a Medicare enrollee must be:
- Homebound (unable to leave home under normal circumstances, short and infrequent outings, most outings would be for physician appointments, but also includes, religious services, and/or attending a special event). At initial visit, Home Health Advantage will meet with patient to assess for homebound status.
- Need intermittent skilled nursing care, physical or speech therapy, or continuing occupational therapy.
- He/she must be under a plan of care established and periodically reviewed by a physician.
